Care Planning & Key Working

This course is designed for Care Teams who need to gain an understanding of the Care Planning & Key Working process.

Synopsis

This 1 day course is designed for Care Staff and First Line Managers and will clarify the purposes and principles of Care Planning and the links between care Planning and Key Working. Delegates will develop their skills and gain a basic knowledge in the four components of Care Planning, they will be enabled to Assess, Plan, Implement and Evaluate Care, and explore the role of the Key Worker.

Aim

The aim of the course is to provide participants with basic, working knowledge in the principles of Effective Care Planning and explore strategies for efficient Keyworking

Objectives

1. Understand what Care Planning is
2. Identify models of Keyworking
3. Be able to compile / interpret a Care Plan
4. Identify ones own role in this process
5. Recall the four main principles of the Care Planning process
6. Acknowledge Care Plans as a necessary and effective working tool
